There are three levels:
- Facility form (to be completed once in the study period);
- Survey level form (to be completed on every day the survey is conducted);
- Consultation-level form (to be completed for every eligible consultation).
Additionally, the Antibiotic Availability form (excel) is completed once in the study period.
